Step 1: To schedule an appointment with a participating provider, call toll-free at 1-800-290-0523 or visit our online provider search.

Step 2: Call to make an appointment with the participating provider. You must show your ID card at the time of visit to receive your discount.

Step 3: You are responsivle for the total bill, less the applicable savings, at the time service is rendered.
